Fees and Insurance
I am an in-network provider with BCBS and Aetna insurance. I also accept out of network benefits. Out of network benefits allow you to see therapists that are out of network of your health insurance. In order to use out of network benefits, you will need to first contact your insurance company to see if they offer out of network benefits for mental health. You will also need to ask them about any deductible or co-insurance with your out of network plan. If you plan to use out of network benefits, you will pay for your sessions upfront. I will provide you with a “superbill” which you can submit to your insurance, and your insurance may be able to reimburse you for part of the psychotherapy session cost. Contact your individual insurance company to find out more about out of network benefits. Please contact me to discuss options for private pay if you prefer not to use insurance. The benefits of private pay include added confidentiality as insurance is not involved as a third party, as well as increased autonomy over psychotherapy sessions.
Good Faith Estimate
As a health care provide I am required by law to supply this notice about the Good Faith Estimate.
Standard Notice: “Right to Receive a Good Faith Estimate of Expected Charges” Under the No Surprises Act
(For use by health care providers, facilities, and providers of air ambulance services no later than January 1, 2022)
Instructions
Under Section 2799B-6 of the Public Health Service Act and its implementing regulations, health care providers1 and health care facilities are required to inform individuals who are not enrolled in a group health plan, or group or individual health insurance coverage, or a Federal health care program, or a Federal Employees Health Benefits (FEHB) program health benefits plan (uninsured individuals), or not seeking to file a claim with their group health plan, health insurance coverage, or FEHB health benefits plan (self-pay individuals) in writing (and may also provide it orally, if an uninsured (or self-pay) individual requests a good faith estimate in a method other than paper or electronically), of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
This form may be used by the health care providers and facilities to inform uninsured (or self-pay) individuals of their right to a “Good Faith Estimate” to help them estimate the expected charges they may be billed for receiving certain health care items and services. Information regarding the availability of a “Good Faith Estimate” must be prominently displayed on the convening provider’s and convening facility’s website and in the office and on-site where scheduling or questions about the cost of health care items or services occur.
To use this model notice, the provider or facility must fill in the blanks with the appropriate information. HHS considers use of the model notice to be good faith compliance with the good faith estimate requirements to inform an individual of their rights to receive such a notice. Use of this model notice is not required and is provided as a means of facilitating compliance with the applicable notice requirements.

You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost
Under the law, health care providers need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided.
You have the right to receive a Good Faith Estimate for the total expected cost of any health care items or services upon request or when scheduling such items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
If you schedule a health care item or service at least 3 business days in advance, make sure your health care provider or facility gives you a Good Faith Estimate in writing within 1 business day after scheduling. If you schedule a health care item or service at least 10 business days in advance, make sure your health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after scheduling. You can also ask any health care provider or facility for a Good Faith Estimate before you schedule an item or service. If you do, make sure the health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after you ask.
If you receive a bill that is at least $400 more for any provider or facility than your Good Faith Estimate from that provider or facility, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate and the bill.
